The Leaving Well Coalition
The Leaving Well Coalition exists to ensure every person in Utah has the opportunity to live well to the end of life by sharing the conversation about their values, making their wishes known and receiving the end-of-life care they desire.
The Leaving Well Coalition grew out of two decades of effort by organizations, agencies and consumers that are concerned with the challenges of end-of-life care. The goal is to develop Utah as a place where conversations about the end of life are comfortable and complete and where providers, patients and families are prepared to help all people experience the end of life they desire.
The focus of the Leaving Well Coalition includes encouraging end-of-life conversations in Utah, sustainability of the Electronic Physician Order for Life Sustaining Treatment (ePOLST), and oversight of the Leaving Well website.
